Pay Schedule:

All verified funds received by Friday close of business will be paid on the following Friday (excluding holidays).

All Check donations over $1000 might be held for verification

All Foreign Checks/Cash will be held for conversion

All Insufficient funds/NSF or additional banking fees will be the responsibility of the ministry or individual.

All funds paid in error will be recovered the following pay schedule, unless previous arrangements have been made.

MEI is a Texas non-profit 501(c)(3) charity and reserves the right to deny services.

United State IRS code requires all tax-exempt organizations to have final authority over all donations.

You and your organization cannot receive donations on behalf of an individual or ministry. Staff or other individuals must fill out an application of their own.

Permission to Initiate Credit / Debit Entries

I hereby authorize Mission Enablers International (MEI) to initiate credit entries and to initiate, if necessary, debit entries and adjustments for any credit entries in error to my (our) account indicated below and the financial institution named below to credit and/or debit the same to such account. I (we) acknowledge that the origination of ACH transactions to my (our) account must comply with the provisions of U.S. law.

Termination by Written Notification

This authorization is to remain in full force and effect until MEI has received written notification from me (us) of its termination in such time and in such manner as to afford MEI a reasonable opportunity to act on it.

Compliance Agreement

By submitting your application and authorization, you are agreeing to the terms and conditions of this application and Mission Enablers International requirements. I (we) understand our obligations and consent to comply with all regulatory, legal, and internal changes, updates and adjustments that the Board of Directors might implement.

Penalty of Perjury

Under penalty of perjury, I confirm that the information submitted in this form is true and accurate to the best of my knowledge.

Non-Profit Acknowledgement

By submitting your application and authorization, you acknowledge that MEI is a Texas non-profit organization that is tax exempt under I.R.C. § 501(c)(3) and MEI exercises complete discretion and control over all funds received and reserves the right to deny services and allocate funds toward any activity consistent with its purpose.
